Wireless Services Agreement

The Agreement Between You and the Amity International, Inc. (Amity Group)

We are pleased to provide you with a wireless phone and wireless services in conjunction with your request that we obtain a phone number for the phone and have it activated for your use, for your convenience, and enjoyment. The low price you paid for your wireless phone and wireless services was made possible in part because, AMITY INT’L, INC. (AMITY GROUP) is acting as the sales agent for the services provider you have chosen, and will receive a sales commission, or free cell phone in connection with the opening for your account. If you had purchased your wireless phone without subscribing the service plan, you would have paid a higher price of the full manufactured suggested retail price.

Amity Int’l, Inc. will be required to repay its sales commission to (“Service Provider”) ZUMA, Verizon, Cingular, and T-Mobile if you violate any of the following conditions:

During the next 180 days, which begin from the day your phone was activated.

1.   You discontinue or interrupt service with (“service provider”) for any reasons (including loss or theft of your telephone, non-payment or cancellation.)

2.   You downgrade your service plan with “Service Provider” during the contract term.

3.   You transfer your service to any other person.

4.   You discontinue service on any other wireless phone account presently activated on the “Service Provider” system.

5.   Your convert your mobile phone area code to anywhere outside of your Home Calling Area.

If any of the above occurs, subscriber or his / her guarantor agrees to pay $300.00 to Amity Int’l, Inc. upon demand. In the event the subscriber violates this agreement cardholder (subscriber or guarantor) authorizes Amity Int’l, Inc. to charge such amount to the credit card account identified on separate paper. If your credit card account is canceled for any reason during the terms of this contract cardholder will notify Amity Int’l, Inc. and provide authorization to change on another valid credit card account.

By signing or reading this agreement, cardholder consents that subscriber is purchasing the wireless phone and wireless services for own personal or business use. If legal or collection action is required to enforce this agreement, Amity Int’l, Inc. will be entitled to recover in addition to all other sums due, such as reasonable attorney’s fee and cost, whether or not suit is filed. According to laws governing credit information, credit protection and Amity Int’l, Inc. can report cardholder’s name and the amount owned to local and national credit bureaus as due and unpaid balance.

By singing or reading this form I authorize Amity Int’l, Inc. to charge my credit card, if the card holder has violated the above 5 restrictions.
